If you know of someone that works in the community, please forward this to them. It’s a statewide survey.
The Louisiana Office of Public Health and the Louisiana Community Health Outreach Network (LACHON, a group that supports community health workers) are conducting a survey of community health workers.
Community health workers have lots of different job titles such as outreach worker, community health educator, community representative/coordinator/liaison, patient navigator, and patient representative. What they have in common is that they are trusted members of the communities they serve. Their job activities usually include doing outreach, education, advocacy, and/or case management to help people and communities improve their health.
If you believe you are a community health worker, you may choose to take this survey. It should take about 10 minutes to complete and your answers will be anonymous: https://www.surveymonkey.com/r/LACHON2017
